Student Evaluation of the Service Learning Experience

This is an evaluation used to gather information from students about their service learning experience in the specific class. It can be sent to students in the last weeks of the semester and the information that it collects can be used to enhance the service learning experience for future students.

Supervisor Evaluation of the Service Learning Student

This evaluation can be used by agency supervisors to assess their service learning students. Supervisors will rate students on attributes associated with the following categories: Knowledge, Performance, Professional Relationships, and Use of Supervision.

These custom evaluations will be delivered to the specified email address making it easy to forward them on to your students and supervisors.
