Certificate Application
Applications for the Certificate in Institutional Research are accepted on a continuous basis. However, it is recommended that students begin the program in the summer with the Introduction to Institutional Research course.
To apply to the Certificate in Institutional Research program, please fill out the application form below and email it to the Program Coordinator.

Registration and Course Enrollment
After acceptance into the program, students not currently enrolled at Florida State will enroll and register as “Special Students” through the Academic & Professional Program Services (APPS) department. Admission as a “Special Student,” does not admit the student as a graduate student at Florida State. However, the courses taken will appear as graduate-level credits on the student’s official transcript. Students may transfer up to 12 credits of coursework taken as a “Special Student” into a graduate degree program at FSU, depending on departmental requirements.
Forms for registration and course enrollment can be found on the APPS website. Students will need to submit the “Special Student Biographical Form” and “Residency Affidavit” for enrollment (note that the “Health History Form” DOES NOT need to be submitted). Students will submit the “Course Registration” form each semester for registration in courses. Forms should be faxed to:

Students already seeking a degree at FSU and state employees using state tuition waivers will register for the on-campus section of courses according to stated procedures. Current students should register for courses during open registration. State employees must submit forms in accordance with the guidelines posted on the Registrar’s website.
Course Fees
Each course offered costs $1000 (includes fees) for off-campus students. There
are no additional fees associated with application to or enrollment in the
program. Students may incur costs associated with the purchase of
books/materials for courses.
Certificate Completion
Upon completion of the Certificate, students will submit a completed Program of Study form and Demographic Form. Forms will be processed by program staff and submitted to the registrar so that a notification of completion of the Certificate in Institutional Research can be placed on the transcript. Students will also receive a paper certificate confirming their completion.
