PROGRAM OF STUDY FORMS

Modified Degree Program of Study is to be submitted to the Graduate Program Coordinator. Just bring one copy on white paper. All other copies on salmon paper would be made at the Department. Currently, there is a copy of Modified Degree Program of Study on reserve in the LRC (124 STB, computer labs). Please save the template on your floppy disk or a computer hard drive before you make any changes.

Certificate in Program Evaluation Program of Study is to be submitted to the Certificate Program Coordinator. You need only one copy on white paper. You can download the copy in both format:

Certificate in Human Resource Development Program of Study is to be submitted to the Certificate Program Coordinator. You need only one copy on white paper. You can download the copy in both format:

Certificate in Educational Policy Program of Study is to be submitted to the Certificate Program Coordinator. You need only one copy on white paper. You can download the copy in both format:

Master of Science (M. S.) Degree Program of Study is to be submitted to the Graduate Program Coordinator. Just prepare one copy on white paper. All other copies on green paper would be made at the Department. You can download the copy of the POS in Microsoft Word format. You'll be able to save an appropriate template on your disk or a computer hard drive.

Specialist's (Ed. S.) Degree Program of Study is to be submitted to the Graduate Program Coordinator on white paper. All other copies on yellow paper would be made at the Department. You can download the copy of the POS in Microsoft Word format. Also, there is a copy of Ed. S. Degree Program of Study on reserve in the LRC (124 STB, computer labs). Please save the template on your floppy disk or a computer hard drive before you make any changes.

Ed. D. Degree Program of Study is to be submitted to the Graduate Program Coordinator on white paper. All other copies on pink paper would be made at the Department. You can download the copy of the POS in Microsoft Word format. Also, there is a copy of Ed. S. Degree Program of Study on reserve in the LRC (124 STB, computer labs). Ph. D. Degree Program of Study is to be submitted to the Graduate Program Coordinator on white paper. All other copies on blue paper would be made at the Department. You can download the copy of the POS in Microsoft Word format. You'll be able to save an appropriate template on your disk or a computer hard drive.
