In order for your proposal to be successful it must demonstrate strong potential for enhancing physical fitness opportunities for your students, and reducing the number of youth that will use or currently use tobacco products. If you and your district are interested in applying for a grant under this program, please read the remainder of this RFP very carefully. The AHA Youth Fitness and Tobacco Prevention/Education Program will provide grants in the following three categories:

  1. Capacity Grant: Support the development of entirely new programs within the school(s) for increasing opportunities for students to participate in tobacco prevention/education and physical fitness activities.
  2. Demonstration Grant: Support the refinement of an already existing exemplary program of tobacco prevention/education and physical fitness that promotes sound practice and replication in other schools and districts.
  3. Innovation Grant: Support the conceptualization, design, development and implementation of tobacco education prevention and fitness programs that are entirely new and innovative, and demonstrate great promise for replication.

Applications from Individual Schools
A school may receive a grant for up to $25,000. Applications of this type may be submitted by one or more teachers within the same school and can address the criteria of any three aforementioned grant categories.

Application from a Consortium
Two or more schools may combine efforts and submit a proposal for implementing a physical fitness program at each of the schools involved. A strong consortium is one in which a manageable number of members have been carefully selected to accomplish specific objectives within the proposal. The schools may be within the same school district or you may partner with a school from another district whose ideals and philosophy are similar to yours. Consortium proposals, if selected, may receive up to $50,000.

Proposal Evaluation
Proposals will be evaluated by a team which will include a district administrator in health and/or physical education, a college professor, a health and/or physical education teacher, and a state agency representative. They will award points as indicated in each section of the Request for Proposal.

 

Application Process and General Directions

  • All proposals must be typed using a font no smaller than 12 point!
  • Follow directions carefully and completely for each section. (Lengthy narratives exceeding the space provided for response will result in a loss of points.)
  • A selection committee will review and score proposals. At least three (3) different people will score each proposal.
  • Administrative Support: Each grant proposal submitted must be accompanied by a letter of support from the participating school's principal. This letter should verify that the principal has read the proposal and is in agreement with the proposed activities stated therein. The principal's letter should also indicate support for the continuation of the program beyond the funding period. If more than one school is involved, as in an application for a consortium, letters of support must be received from each school's principal.
  • To be competitive, the proposed project must clearly demonstrate strong potential to be replicated in other schools of similar grade levels and demographics.
  • Be sure that all sections have been completed.

Developing the Grant Proposal [Use this section as a guide when completing the application.]

Section I ---- School Information --- (5 Points)
Complete each question with the appropriate response in the blank provided.

Section II ---- Grant Category & Project Description --- (20 Points)
Complete each question within the space provided unless otherwise indicated. Longer answers will result in a loss of points.

Question 1: Explain why there is a need for physical fitness and tobacco Prevention/Education. (e.g., The project may improve student health, academics and behaviors.)

Question 2: You might include such things as grade levels, racial mix, number of students on free and reduced lunch, etc.

Question 3: Describe each person's qualifications and amount of time they will be working with the project.

Question 4: Describe what students and staff will be doing. (e.g., intergenerational and/or student mentoring, peer facilitation, wellness plans, etc.)

Question 5: List only those standards and benchmarks that apply to your project.

Question 6: These initiatives were developed by student members of the SWAT teams across the state. Examples of each category are as follows:

- Sports Sponsorship by Big Tobacco: Contacting race car drivers and asking them not to use tobacco companies as sponsors.

-Tobacco Industry Image Improvement Campaign: Contact principals and ask them to carefully look at free materials from tobacco companies and discard them.

-Political Contributions by Big Tobacco to Candidates: Ask local Senators and Representatives no to accept Big Tobacco money.

-Spit Tobacco: Ask school athletes (especially baseball) to speak against or write professional players not to use tobacco products.

-Tobacco Use on School Campuses: Work with school board on the tobacco policy to free all property of cigarettes, cigars and spit tobacco.

-Increase Awareness of Tobacco's….: Have students investigate what other companies the Big Tobacco companies own.

Question 7: Describe how you will integrate these physical fitness and tobacco prevention/education in your project.

Question 8: Describe where the project will be located and where most activities will be conducted. Include areas off campus, if appropriate. (Example - at the community park or community center, community college or university, etc.)

Question 9: Describe the existing building, sports equipment, electronic equipment, etc. that will be used to support your project.

Question 10: Check times that are appropriate.

Question 11: Give examples such as promotion through newspaper articles, presentations to school board, county commissioners, letters to legislators, etc.

Question 12: List any additional funding which you will use to support and sustain your project beyond the funding period. (Example - from your school, district office, private and/or community support, business partners, etc.)

Section III ---- Project Objectives --- (10 Points)
Your objectives should address performance outcomes in your students relative to their participation in physical activity and how this participation and the project as a whole will strive to prevent or reduce tobacco use by students At least one objective must have and tobacco prevention/education emphasis.

Section IV ---- Project Strategies/Activities--- (20 Points)
List your objectives again. Under each objective, use the chart to create strategies/activities to assist you in attaining the objective. List as many as needed. Assign the person who will be responsible for each objective under the "Who will do it?" column. Assign a date by which you plan to complete strategy/activity under the "By When?" column. Be sure that all questions under Section II are addressed. Copy the chart again if you have more than three objectives.

Section V ---- Project Partners --- (10 Points)
List any community organizations that will assist you with your project. You are required to work with your county Tobacco Coalition and the Students Working Against Tobacco (SWAT) program. Include a letter of support from this agency as well as any other agencies provided in the appendix who are providing support. Be sure to find a partner(s) who will assist with the continuation of your project. Include a letter of support from your partner(s) in the appendix. (Examples - health insurance company, hospital, HMO, local businesses, etc.)

Section VI ---- Additional Information --- (5 Points)
Discuss aspects of your project that you feel will help reader to better understand what you plan to do. Include any innovative strategies you will implement.

Section VII ---- Project Budget--- (10 Points)
Budget Summary: Complete the chart. You need not use all categories.

Budget Narrative: Provide a brief, general explanation of how the dollars in each category listed will be spent and for what purpose. For example, salary funds will be used to pay for teachers to work overtime with the students; equipment funds to purchase heart rate monitors and computers to print out heart rate monitor results.

Equipment for this project may include computers, fitness machines, heart rate monitors or other appropriate items necessary to complete the stated project objectives.

Section VIII ---- Evaluation --- (20 Points)
The University of Miami will provide an evaluation of these projects in conjunction with the Florida Department of Health's Division of Health, Awareness and Tobacco. You will be required to participate in this evaluation. Answer each question and specify the name of any additional evaluative instruments or methods you will use to help evaluate your project.
