Request a graduate application from the Office of Graduate Admissions, Room 2249, University Center, Florida State University, Tallahassee, FL 32306-1009; telephone (904) 644-3420. Follow instructions below.
The Office of Graduate Admissions Requires:

A completed graduate application, returned to the Office of Graduate Admissions [address above].·
Two (2) official transcripts of all previous college work.·
A copy of your Graduate Record Examination (GRE) score, requested from Educational Testing.
NOTE: The GRE is required by the university for admission. General FSU Graduate Admissions calls for a GRE combined verbal-quantitative score of at least 1,000. However, if your score is below 1,000 and you have a 3.00 GPA in the last two (2) years of undergraduate school, your application can be considered.
Students not meeting these criteria may, on rare occasions, be accepted on the basis of promise in their respective careers, letters of recommendation, educational and vocational background, or as part of our Affirmative Action Program. Prospective students may be required to attend an on-campus admissions interview at the discretion of the faculty.
The Department of Human Services and Studies Requires:

A current resumé.·
Three (3) letters of positive recommendation for graduate work.·
An autobiographical statement (two to four pages), including reasons for pursuing an advanced degree at this time, a statement of your career goals, and a little personal history.·
For master’s-level applicants only, a copy of the completed in-house application. [The FSU application alone is used for the Ph.D. level.]

Financial Aid: For general student financial aid, contact the FSU Student Financial Aid Office, Room 2466 University Center, FSU, Tallahassee 32306-1023, (904) 644-0539. Departmental financial aid is also available on a limited basis.

The application deadline for Ph.D. candidates is January 6th.
The application deadline for M. S./Ed.S. candidates is March 1st.
The deadline to apply for fellowships/assistantships is January 6th.
